Severe over pronation has caused there to be an exostosis (boney growth of bone). Your posterior tibial ligament may have collapsed; this is the ligament that holds the arch of the foot in place. This is caused due to excessive pronation and it also lead to pain at the top of the foot due to the structures of the foot being weakened. The conservative options are non-steroidal anti-inflammatory, ultra sound, insoles and ice foot wraps. Surgical option includes the release of the tendon from the muscle belly proximally to distally around the medial malleolus and the insertion point on the navicular. Any tenosynovium is removed and any tearing within the tendon sutured.
