I stepped on a hard plastic ice cube. One of those joke cubes that have a bug in them (my son's). It got me dead center in the ball of my foot. A month has passed and the pain hasn't gone. Although it doesn't bother me terribly when I wear shoes, barefoot the pain is excruciating. There is no mark(never was) and I don't percieve any swelling. It is very tender to the touch.

Superficial damage would have healed within a month. Sounds like you have damaged a tendon, visit a chiropodist for a full examination.
